WHAT GREEN LOOKS LIKE:

  • CONSISTENT MOVEMENT: You are experiencing your usual level of resting tremor or stiffness, but it isn't rapidly worsening.

  • FUNCTIONAL INDEPENDENCE: You can manage daily tasks like dressing and walking with your usual level of support.

  • CONTROLLED NON-MOTOR SYMPTOMS: Mild constipation or minor sleep changes are present but are being managed through diet and routine.

YOUR ACTION PLAN: Maintain your routine. Keep up with your exercise, stay hydrated, and continue taking your medications exactly as prescribed. Our nurses focus on this zone to ensure your environment is safe, trip-hazards are removed, and your care plan is perfectly aligned with your GP's recommendations.

YELLOW ZONE: THE WARNING PHASE

The Yellow Zone is where "monitor and mention" becomes "call the nurse." This zone represents a change from your baseline. It is not an emergency yet, but it is a signal that your current care plan may need adjustment to prevent a hospital visit.

WHAT YELLOW LOOKS LIKE:

  • INCREASED FALLS OR "NEAR MISSES": You are feeling more unsteady or have experienced a stumble.

  • MEDICATION "WEARING OFF": You notice symptoms returning before your next dose is due.

  • SWALLOWING DIFFICULTIES: Occasional coughing while drinking or taking longer to eat.

  • MOOD CHANGES: Increased anxiety, apathy, or new vivid dreams.

YOUR ACTION PLAN: Reach out now. When you notice these shifts, it is time to contact your clinical team.   • SUDDEN IMMOBILITY: A major "freeze" or inability to stand or walk at all.

  • SEVERE CHOKING: Significant difficulty swallowing or food "going down the wrong way."

  • ACUTE CONFUSION: Sudden, severe hallucinations or distress that is out of character.

  • INABILITY TO TAKE MEDS: Vomiting or an inability to swallow vital Parkinson's medication.
